Local 1981 has made available to our members the most frequently used Forms for State Employees.  We have attached in PDF versions of the State Employee Grievance Form, the Grievance Fact Sheet, District Council 89 Grievance Questionnaire and the Witness Statement.  Additionally, we have provided our Local's Donation Form, which is used to set up a Donation program for fellow union brothers and sisters in their time of need.
