If you would like to set up a Donation Fund for a Member, Here is the Form which must be filled out and submitted to Local 1981.
It will be reviewed and approved or denied. Please note that this Donation Form must be accompanied by a Letter on Original Letterhead from the organization which will be setting up the fund.
